We provide Supported Living, Domiciliary Care, 24-hour Live-in Care, Respite Care and Outreach Support.

Our services cater to the needs of adults (18 - 65 year olds) with a range of difficulties. We create care plans around the service users so do not hesitate to contact us to discuss the service(s) that suits your needs.

Supported Living


Our Supported Living schemes are designed for those who want to live as independently as possible, while still being able to rely on access to the individual support they need. This service is ideal for those with capacity and ability but still need help with day-to-day tasks and activities. We accept referrals from local organisations such as Adult Social Services, Hospitals and individuals who have been released from custody and are under the support of the Mental Health Teams. People with profound or multiple complex needs can also benefit from a Supported Living environment as the care staff provide 24-hour support and provide a personalised support due to
the small setting of such service.

Domiciliary Care


This service is designed for people needing support with their day-to-day tasks at home. Visits are arranged at times that fits into your routine, care workers can be available when you wake up to help you get washed and dressed, or to help you with meals throughout the day. Domiciliary Care visits are flexible, so whether you need daily or weekly visits, a care plan can be created around your needs.

Services Include (but not limited to)

  • Washing and dressing

  • Assistance with getting in & out of bed.

  • Manual handling

  • Managing commodes

  • Continence care

  • Toileting

24-hour Live-in Care


For those that require 24-hour care, we provide personalised, one to one live-in support and companionship in your own home. You may feel that you could benefit from the comfort of knowing that there is always someone at home to provide you with support. Our live-in care workers adapt and work to support you in your own home, so you can keep your independence. Live-in care is also a preferred option alternative to going into a care home. We work with all parties involved to decide on the level of care you need and we can create a suitable care plan. The care workers can help with housekeeping, cooking, continence care, mobility and much more.

Respite Care


If a current care giver (care worker, support worker, family member or current placement/agency) requires a break, we are here to ensure your care responsibilities will be taken care of. We can arrange to provide service for the period required and work with all parties involved to understand the care services that is provided and any other requirements to ensure that the transition is as seamless as possible, minimising any disruptions.
 

Outreach Support


Whether someone needs only a few hours each week, or a longer period to work towards a specific objective, our well trained and experienced staff are in place to support service users to achieve their goals. The Outreach service is tailored to suit the needs of the people we support and provided at times that suit them.


Outreach support includes:

 

  • Help to develop and maintaining life skill (food shopping, cooking etc).

  • Support with finances and budgeting Supporting

  • Supporting individuals to access college and voluntary or paid employment.

  • Accessing the community for activities. (community centre)

Psychologically Informed Environments


The core to our service delivery is the development of our Psychologically informed environments (PIEs), at its most basic it is an environment that makes use of methods, which are informed by psychological theories and frameworks.


This approach

​

  • Reduces challenging or aggressive behaviour.

  • Increases positive behaviour by increasing self-worth, self-belief, and hope.

  • Ensures service users maintains their full tenancy by reducing abandonment and evictions.

  • Achieves the most positive outcome for each individual as they identify their own goal or ambition, and direct how they would like to use our support in order to achieve that outcome.
